INSURANCE

Insurance, deductibles, and other non-covered expenses are the responsibility of the patient at the time of service.

For claims to insurers with whom we do not participate, you will be provided with a superbill so you may file the claim. You should retain one copy of the superbill for your records and submit one to your insurer with a claim form.

• Medicare as a service to our patients, we accept assignment of Medicare claims, a co-payment may be required.
• Medicaid We accept only Virginia Medicaid. Co-payment may be required.

We are participating providers for many insurance companies. Many companies have specific rules requiring pre-approval for testing and referrals. Our insurance staff will be glad to assist you with questions about your coverage.
